Registration Forms '12 - '13 School Year Preschool Registration Form - Use this form to register your child for preschool Toddler Registration Form - Use this form to register your child for the toddler program
All registration forms should be filled out and returned to our director's office or mailed to: Creekside Children's Place
Other Forms The following forms must be faxed (952.926.0045) or returned to Creekside prior to your child's first day of school. Please fill out separate forms for each child. Child Care Immunization Record - Immunization records must be on file for each child in our program. Health Care Summary - A health care summary (filled out by a doctor) must be on file for each child in our program. Alternate pickup authorization - Each child must have this on file. Toddler Permission Slip and/or Preschool Permission Slip - Print and fill out form(s) for each child attending Creekside. Be sure to check the appropriate boxes before signing.

Classes are filled on a rolling
admissions basis. Priority is given to returning families then Good
Samaritan families and then new families.
